В период пандемии рекомендуется избежать начала приема тиопурина или увеличения дозы, что позволит пациентам избежать потенциальных побочных эффектов. Если пациент находится в контакте с человеком COVID-19, необходимо рассмотреть возможность временного отказа от приема тиопурина на 2 недели. Если у пациента положительный результат теста на SARS-CoV-2 и/или у него развивается COVID-19, возможно рекомендовать временное прекращение приема тиопурина до тех пор, пока пациент не избавится от инфекции.
Лечение кортикостероидами (особенно преднизолоном в дозе ≥20 мг/сут или его эквивалент) связано с повышенным риском инфицирования. Однако неясно, связана ли терапия кортикостероидами с повышенным риском развития COVID-19 или ее осложнений. Данные о будесониде , кортикостероидах местного действия с низкой системной биодоступностью показывают, что эти препараты связаны со значительно меньшим количеством побочных эффектов по сравнению с системными кортикостероидами и их побочные эффекты приближены к плацебо.
The use of thiopurine (azathioprine and mercaptopurine) reduces the immune response to viruses, which is associated with an increased risk of opportunistic infections. There is limited evidence that they increase the risk of respiratory infections. The risks and benefits should be considered, but most patients can continue on a stable dose. In patients in stable remission, elderly patients and in the presence of concomitant pathology, it is recommended to stop taking thiopurine. During a pandemic, it is recommended to avoid starting thiopurine or increasing the dose, which will allow patients to avoid potential side effects. If the patient is in contact with a COVID-19 person, temporary withdrawal of thiopurine for 2 weeks should be considered. If a patient tests positive for SARS-CoV-2 and/or develops COVID-19, temporary discontinuation of thiopurine may be recommended until the patient clears the infection.
It is the transition to the use of IPV in Ukraine for all doses of the Immunization Schedule that should be the next task for vaccination. For this, several mechanisms can be used, including humanitarian assistance, the conclusion of planned direct contracts with vaccine manufacturers based on the needs of the required composition of vaccines according to the Calendar, in particular, taking into account the existence of modern combined vaccines with IPV, which can also be used at an older age. In the absence of combination vaccines with IPV, IPV can be used as a single vaccine.
