Responding to such epidemic challenges is very difficult both organizationally and financially. The result is evaluated over time with well-organized epidemiological surveillance. According to the approved polio outbreak response plan, which arose in early October 2021, it was only possible to start the 1st round of the campaign on February 1, 2022. It was supposed to last 3 weeks and was aimed at vaccinating 140,000 children from 6 months of age with IPV. up to 6 years who missed vaccination according to the Calendar. Almost by the end of the 3rd week of the 1st round, as of February 18, 2022, only 28% of the planned number of children were vaccinated. Therefore, we decided to extend the 1st round for an additional 2 weeks, which again was not performed due to the outbreak of war. Thus, there was no actual response to polio outbreaks.
Both patients and doctors should be well aware of the need for long-term therapy with periodic monitoring of key indicators and the obligatory achievement of the so-called target levels of blood pressure, that is, stable normalization of blood pressure. Only such an approach to the treatment of patients with hypertension can give a successful result. But at present, as mentioned above, only a very small number of patients with hypertension are treated effectively. In this regard, there is no decrease in the frequency of complications of this disease in Ukraine.
Considering that a similar picture took place in previous years, in Ukraine there was an accumulation of a layer susceptible to poliomyelitis among the child population. In addition, since April 2016, the oral (live) polio vaccine (OPV), which is used in Ukraine, since the 3rd vaccination, does not contain type 2 poliovirus (two-component OPV, which is now used worldwide, contains only polioviruses). ). type ov1 and 3). This also contributes to a decrease in both individual and population immunity to type 2 poliovirus. The result was an outbreak of poliomyelitis caused by vaccine-derived type 2 poliovirus (a vaccine-derived virus that acquired neurovirulent properties during circulation in a low immune population.). There was no adequate response to this outbreak, namely additional vaccination rounds for children under 6 years of age to stop the circulation of the vaccine-related virus. In January 2022, a new outbreak emerged.
Еще в 2002 г. по обоснованию Института в Календарь прививок Украины была введена инактивированная полиомиелитная вакцина (ИПВ) для 1-й прививки против полиомиелита (остальные 5 доз по возрасту дети продолжали получать ОПВ). В 2006 г. – эту вакцину начали применять и для второй прививки. Это позволило сначала снизить, а затем практически прекратить случаи вакциноассоциированного паралитического полиомиелита в Украине. С тех пор неоднократно поднимался вопрос перехода в Украину на полную схему вакцинации ИПВ, как это практикуется уже много лет во всех странах Европы, США, Канаде, Австралии. В настоящее время эта проблема приобретает еще большую актуальность, учитывая как эпидемическую ситуацию, так и перспективы вступления Украины в ЕС.
